Self-Declaration Health Form

hi,

You'll find that some of the listing of Items for Application given on the MoTour site have different numbers for the same items when comparing the lists for New Application, Direct Application and Agent Application.

Consistency would be nice, but it's not necessary if you know that it's not consistent. I also don't know why there's 3 different applications (New, Agent, Direct).

The self-declaration for health is a new one for me. Guess it's something generated by H1N1?  It says :

8.   Self declaration on your/ dependants health conditions - Form can be downloaded from MM2H website.

However I can't find the Form. I can find the Form for the Medical Report (signed by a doctor) RB11.

Has anyone found it?

Medical report for mm2h self declaration is Form RB1

The Medical Health Report Form RB11 which part 1 and 2 are for the applicant and part 3 is for the doctor is downloadable from Joy-Stay website.

So it's the usual RB11 Form, just fill in Part 1 & 2. Many thanks for the info.

The form is also downloadable from the www.mm2h.gov.my website, and from this site.

I received this information today from Joy-Stay :

All applicants/dependents must complete the self declaration health conditions on the new RB I Form. I only obtained a hard copy of this new form from the Ministry of Tourism and created a PDF file for my clients to complete. I attach here for your reference. RBI Form is meant for submission purpose and RB II is to be completed only after the applicants obtain MM2H Conditional Approval. The difference between RB I and RB II is RB I does not have the Part 3 which needs to be completed by a local doctor here.

The RB I Form which Joy-Stay created came with a signature and date line, this is to avoid any questions raised by the authorities in the future.
 
Someone mentioned that he could obtain the said RBII form from our website, FYI, the RBII we have on our website is meant for Stage-2. We do not place the new RBI form on our website because the authorities keep changing their rules and regulations and we would only provide the most updated forms to those who are ready to submit their MM2H applications.

Of course I suppose that you could create your own (I confirm that I've never felt better, and don't have anything 'orrible  ). Or email MoTour about the Form which is what I did yesterday.

Joy-Stay has already explained the very recent NEW requirement.
She also accepted my forms as admissable even though they may now not be correct.
I should correct my last post on this subject to align with Joy-Stay's statement.

i find some questions on the health self certification quite amusing esp those pertaining to  one's senses and  Gila Babi (epilepsy) which directly translates as "Mad Pig"
