I've copied this from the motour site so I can be clear on (a) renewal, (b) extension to use up the rest of the 10-year visa, and (c) extension at the end of the 10-years. Yep, there's 3 different categories.

Motour Site 23/05/2007

Renewal of Social Visit Pass Under this programme
    
For participants who joined the programe before 1 April 2006 , renewal can either be submitted directly by the individual participant or through licensed company to Immigration Department , Putrajaya or The State Immigration Office which issued the initial Social Visit Pass.
    
The following documents are required for the renewal :
1) IM 55 Form
2) Copy of the old and new passport.
3) Copy of the fixed deposit certificate
4) Copy of the valid medical insurance.
    
EXTENSION
 
Extensions- Change of Passport
This applies to those who did not receive the full duration of the 10-year Visa because of the short validity period of their passport at the time of approval and would like to receive the remaining period upon the issuance of a new passport.

The applicant must appear in person at the Immigration Department of Malaysia and is required to bring the following documents :

1) Old and New Passport
2) Photocopy of the New Passport
3) IM.12 Form ; and
4) A fee of RM90 per year is to be made payable to the Immigration Department of Malaysia
        
Extensions - After 10-Year Period
The applicants must be present in person at the Immigration Department of Malaysia and is required to bring the following documents:

1) Letter of Intent to continue stay in Malaysia under this programme
2) Passport
3) Photocopy of Passport
4) IM.55 Form
5) Payment of RM90 per year for the social visit pass fee, payable to the Immigration Department of Malaysia

So, renewal for 5-year visa = need medical insurance. Extension for 10-year visa = no need for insurance.

Does it also mean renewal of the 5 yr visa - applicants are not required to write a Letter of Intent to continue to stay in M'sia under MM2H program?

I'm getting a headache trying to figure out if the requirements are randomly different, different with a purpose, or just a typo. When I come to renew my 5-year visa, I'll have a letter of intent, just in case.

I suppose that with all the changes, a lack of clarity here and there is to be expected. But it doesn't help when we have to make special trips, and incur expenses.

I'll email motour, although they may be tired of me. And if I get a reply, I'll post here.

The Letter of Intent is required for the renewal. You have to go back to the Immigration Office you obtain your initial visa to renew it. It will be renewed for 5 years, provided the initial conditions at the time of the application are still intact. For this you will need to show your pension income, or your fixed deposit statement. Concerning the medical insurance, I was told by the motour official that it will not be required, unless you do not have it during your initial application.

If you are now at another state other than the one you initially lodge the application, we at Aubella can help you to request for a change of immigration office, of which your visa can now be renewed at a nearer immigration office, to save some expenses. For example, from Damansara Immigration to Penang Immigration. However, please allow at least 6 months before your renewal for such a request, as Immigration has a lot of issues to handle at the moment, and some process may take quite a while.

I guess you never got a reply on that specific query Scott?
The trouble with taking the ‘middle of the road’ position is that you get run over from both directions.

Nope, no reply. Also it coincided with the period that MoTour was trying to find out who I was, where etc. etc. They were just updating their files, remember? Did anyone else ever get asked?

Anyway I let it all drop.

Nope,

We have had no contact over the past 5 years with any of the MM2H official bodies except for our visit to Putrajaya to renew the kids visa, and then nothing specific was asked/recorded or updated!

I just had to share this with you from another forum I look at that has nothing to do with MM2H or any political views it is just as scuba diving forum and this Malaysian citizen is having a rant

At the end of the day if it happens to them it is difficult for us to complain

About a month ago, I was a bit stupid. Left my passport in my jeans, and off it went into the washing machine. Needless to say, I almost fainted when I discovered that my passport had become "kiam chai" (Pickled vegetable).

This is what happened subsequently- (Note: Was living in S'pore. PR application in progress)

1) went to Malaysian embassy (wah very nice part of Singapore, all the staff condo very nice and "expensive", can I get a job there ah? Oh no, cannot. I don't have connection). Embassy refuse to replace passport, say must go back to JB to do it. (What if I'm in Africa? Also must go back to JB meh?)

2) Ok, so have to apply for emergency passport (valid for single trip back to malaysia, cost RM50)

3) Next day, went to JB immigration. To get a form also must pay RM1!!! Prepared all documents. Went to the counter. They refuse to accept my application.

a) Is it because I'm not a malaysian and don't have my IC??? No.
b) Is it because I was not born in Johor??? No. I was born in Johor.
c) Is it because the address in my IC is not one in Johor??? YES!!!!!!!!!

4) Is immigration department a state agency or a federal agency? Answer: Federal Agency

5) Why cannot submit in JB? Answer: because too many people apply in JB. So we force people to go back to where they come from to submit there.

6) But I AM from Johor and I was born here, it says so in my birth cert!!! Answer: well, it doesn't say so in your IC. 

7) Did I pay my taxes? Is my taxes being used to pay for your salary? Answer: *Blank stare*

8) So if I change my address in my IC to a JB address even though I don't live in JB then you will accept my application? Answer: yes. And we don't want to know that you fake an address on your IC so that you can replace your passport.

9) Pick up the phone and call some people. Next day was taken to a senior officer (dunno what pangkat la, but got a koolie to carry his bag so must be orang besar la.) who signed on a piece of stick-on note. Went downstair and shoved it at the counter. Immediately got the papers processed.

10) Next to the counter, there was a large poster (flow chart) of standard process flow of passport application. I think got ISO9000 also.

11) 3 weeks later, got myself a spanking new passport. There were FIVE counter officers processing collection. In one hour, 5 people managed to collect their passport. And they tried to sell me a passport cover for RM3.

WHAT IS WRONG WITH THIS COUNTRY?

YOU TELL ME. BUT DON'T TELL ME "BIASA LA"
