It's the Simple Form

hi,

When you've got Conditional Approval, and you go to collect the visa you'll need to submit a medical form.

This is available for download from the Ministry of Tourism site: http://mm2h.motour.gov.my. It's Form mmsh/05 - now (15.10/2007) Form RB11.

You need to take the form to any clinic in Malaysia and get the boxes ticked, and the form signed by a registered doctor, and get the Clinic's stamp put on. The charge for this is about RM30. More than RM50 and you're paying too much so check the price at reception before seeing the doctor. Or maybe you don't want to walk around KL checking out clinics for the sake of a few ringgit.

The form only needs for the doctor to establish that you're alive. There are no tests, no physical examination, no x-rays, no nothing. The doctor can sign the form with a clear conscience as long as you're breathing. It takes 10 minutes, longer if you end up chatting with the doctor.

However, most doctors have only heard of the Immigration Medical Report Form in connection with the detailed form required for foreign workers. This one needs blood tests etc. and the charge is about RM150.

Most doctors only vaguely know about my2home, and the simple form. So don't end-up paying for the detailed form.

Thanks Scott, will bear that in mind next week when we go for our medical.......if we can find the nearest clinic to our Hotel in KL (Equatorial) not sure of spelling.

Sometime between posting this topic (09/05/07) and checking today, the designation for the form has been changed from mmsh/05 to RB11.

In fact, the form always was RB11. It then got re-typed and issued, on the MoTour site, as mm2h/05. The form looked good. Now it's back to RB11 which is the less attractive and less professional version.

At least, that's my take on it, not that it matters much. It's just weird, like dejavu. Maybe during one Motour site upgrading the older form got re-linked instead of the newer form.
