Where to sign medical form in KL
« on: March 17, 2010, 07:38:48 PM »
I went to Immigration Unit at 6/F of PWTC to renew my mm2h visa.  A medical report Form RBII is required.  I want this form be signed and stamped by a doctor who will only carry out “visual exam”.  Does anybody know in KL where can I get it done?

Maybe I don’t remember correctly, but it seems that someone mentioned here or there about a doctor near PWTC who was familiar with mm2h and could sign and stamp the form in 5 minutes?  Anyone can help?

hi, hwo,

Welcome to the site.

The medical report form RB11 isn't needed for renewal  ?  ?  ?

A copy of a current Medical Insurance is needed.

I can't yet find the reference to the PWTC doc.

I asked Joy-Stay about KL doctors as Joy-Stay is located in KL.

Their reply was, "We only use the doctors from Klinik Mega at Seri Petaling. The doctors who would complete the RBii form would be Dr. Prince and Dr. Rohada. Tel: 03-90573969. The applicants who wish to see the above doctors must call the clinic to find out the doctors' clinic hours because they are other doctors on duty who are not familiar with the MM2H Programme. "

You could phone, check when these docs will be in, and get the address and how to get there info.

Thank you very much Scott.

I called the clinic and was told that the said doctors would be in tomorrow.  Do you know how much will be charged to complete 1 medical form?

I got the medical form completed at Klinik Mega after undergoing some basic exam like eyesight testing, height & blood pressure measuring, bodyweight taking, and answering the doctor some questions.  The doctor said these were necessary formalities.  I was charged RM30.

Yes, Scott, it is quite unexpected that form RBII is required.  The Immigration Officer gave me the form and asked me to complete it at any clinic.  He said the form had to be included in my set of documents for mm2h visa renewal.

I find that rather disturbing.  So, future scenario - 30 years down the road, I'm renewing my visa for the third time, stone deaf, half-blind, completely ga-ga and in a zimmerframe, will the Doc verify I'm in good health and sign the RBII form?  :-X

Perhaps the test is  """Are you in good health FOR AN 84-YEAR-OLD"""   !!! :o

Well done on getting it done and for RM30.

When I renewed my visa I took along a copy of the requirements listed on the mm2h.gov.my site. This was in case I was asked for anything not on the list. The RB11 form hasn't been a requirement for renewal so far and it still isn't. I guess the officer you met was wrong. But what to do if you're stuck in a government office and the officer says, "well, that's that."

I'll check the situation with MoTour once they've replied to another outstanding matter and post here. Let's see what happens.

Hi, Papaya, you need to go to the same doctor who will also be in a similar state. No problemo.   8-)

When the immi officer asked for the medical report I replied that this wasn’t mentioned on the website.  He said “yes, we need it” and went to the office behind to fetch form RBII for me.

A poster on another forum renewed his visa in December last year.  He applied in Putrajaya Office.  After submission of documents he was asked to come back after 7 days.  7 days later when he came back he was asked to go for a medical check that had not been mentioned before.  He did it in Putrajaya, went back to immi and got the 10-yr visa.

I understand that there is a clinic, Klinik Salma, next to the Immigration Unit of MM2H Centre, on Level 6 of PWTC.  It is not necessary to make an appointment but you should download Form RB II [download pdf here] for each family member,  to be completed by the doctor.

Klinik Salma appears to be far more convenient than Joy-Stay's Klinik Mega at Seri Petaling, which is quite far from PWTC.

Hi Azka,

Do you know what time the Klinik Salam is open please, or

do you have the contact details so that I may contact them to make sure a doctor will be available when we attend.

Hi Jac

Sorry, I don't know anything more. I was provided this information by an extremely reliable source and hope to use it in September when I am planning to pick up my visa. So if you find out more do pleae post it.

You might like to see this post by Danny that mentions Klinik Salma.

Good luck!

Hi,

We hope to complete the formalities for MM2H late August/early September and if there is anything we can share with the forum which will be of help we will certainly do so.

Really looking forward to being in Malaysia again. Juts trying to work out when we will be in a position to finally relocate!

I have the word officially that the RB II form is definitely a requirement now. It must be new, because it's still not on the website. I have a doctor at the Rophe Med Clinic in Kelana Jaya (PJ), and he's quite good. I've been seeing him fairly regularly (I have slightly high blood pressure) for the past six months, and I like him very much. I would recommend the Rophe Med group to anyone, for the form and/or ongoing care, as I've heard they are quite good.

I have a question. I'm 61, and what would be an approximate average monthly cost for hospitalization-type medical insurance in the KL area for me?

hi, russinmy,

Welcome to the site.

Are you posting about renewals?

A few months ago the RB11 form was listed as a requirement for renewal. Then Motour said that it wasn't needed and would be taken off the site. Eventually it was taken off, and this was very recent, and it's still off.

So now it's back as a requirement.

Do you know how much does the doctor you recommend charges?

My mate who just picked up the initial visa (this medical form is listed on the MoTour site) has just got 3 forms signed for himself, his wife and their boy at a total cost of RM45. But that's in Perak rather than PJ. But what a bargain.
